Reservations help us prepare for each visit. Once you submit, we'll reach out to confirm your time, location, and what to expect.

  • 3Complete your intake forms before the visit — we send the link in your confirmation.
  • 4Show up (or log on for telehealth). Payment is collected at your appointment.

Our Focus. Patrick Smith, ND, BCHHP and Sovereign Health specialize in natural health treatments, functional testing, and personalized wellness protocols. We do not diagnose or claim to cure disease, and we do not prescribe pharmaceutical medications — our work is centered on natural, root-cause approaches to health.

Voluntary Participation. I am voluntarily choosing to work with Sovereign Health for natural health support. I agree to share any relevant health history, conditions, or medications with my practitioner so we can build the most effective plan for me.

Medical Emergencies. Sovereign Health is not an emergency service. In case of a medical emergency, I understand I should call 911 immediately.

Agreement. I have read and understand the above, and agree to it freely. If completing on behalf of a minor, I am their parent or legal guardian.
